Vineyard Care is a service provider with a focus on opportunities for rehabilitation of persons with mental disorders. We provide personal care in community settings that promote and allow individuals to flourish. We also work with individuals with comorbid alcohol and drug use; to promote gradual reduction and harm minimisation for their underlying substance use disorders where identified as contributing to mental health relapse.

We undertake regular assessment of individuals in the community and work closely with commissioned providers to promote access to community or inpatient rehabilitation. We deliver personal care to individuals during the time of their community rehabilitation, where individuals have severe and enduring mental illness requiring aftercare, as per Mental Health Act Section 117.

Vineyard Care works with housing providing partners to ensure our residents not only have aesthetically pleasant but also clinically safe environments which take into consideration their historical risks ( See our referral form). We also work to provide personal care within supported housing and also provide additional support to patients with comorbid substance use affecting; a) their compliance to medication for their mental illness, b) ability to maintain personal hygiene, c) ability to maintain cooking and balanced dietary needs and d) maintaining wellness focused life routines.

We seek to take into consideration high risk symptoms which are regularly reviewed by our nominated individual (consultant psychiatrist) as part of a medical liaison protocol, engaging medical colleagues as part of an enhanced psychosis pathway.
